Meyer Care Management
A Meyer Care Manager will complete a comprehensive assessment identifying the level of physical assistance needed in all areas of activities of daily living, including bathing, dressing, grooming, nutrition and mobility. The assessment also identifies medical conditions that may impact a person's level of health and wellness. The Care Manager then begins to develop a Plan of Care to address the care needs identified.
A Meyer Care Manager can help manage medical appointments and connect you with specialists when needed. A Care Manager takes the extra time to explain medical diagnoses, procedures, and medications prescribed by your physician, so you can feel confident about your healthcare. A Care Manager serves as your Advocate and will assist you in times of illness to navigate the healthcare system.

The founder of Meyer Care Management, Darin Meyer is a Registered Nurse and Certified Care Manager that has worked in healthcare for over 25 years. During his professional career, Darin has worked in hospitals, longterm care communities and home health agencies. A former Army Nurse, Darin's strong leadership and management background has lead teams to Deficiency Free Surveys in Continuing Care Retirement Communities. As the Vice President of a Home Care and Home Health agency, Darin hired, trained and managed a caregiving staff of over 300 professional and paraprofessional caregivers.
